1. Services obtained for emergency conditions from non-affiliated physicians
and hospitals
2. Injuries, infections, or conditions resulting from:

• alcoholic liquor intake
• unauthorized use of prohibited drugs or regulated drugs
• violation of law or ordinance or unnecessary exposure to imminent danger,
• direct or indirect participation in the commission of crime,
• direct participation in riots, strikes, and other civil disturbances,
• war or any combat-related activities while in military service,
• extreme sports,
• developmental disorders,
• circumcision and sex transformation,
• tattoos,
• body piercing,
• attempted suicide,
• acupuncture,
• congenital, genetic, and hereditary conditions,
• chronic dermatoses

3. HIV and AIDS-related diseases, sexually transmitted diseases
4. All pregnancy and maternity-related conditions
5. Epidemic or pandemic infectious diseases
6. Psychiatric and/or psychological disorders including consultations with Maxicare mental health experts
7. Aesthetic, cosmetic, and reconstructive surgery
8. Oral surgery and dental treatments
9. All types of vaccines except for special offers extended by Maxicare to its members

How to Activate Your Maxicare PRIMA Silver

After purchase, you need to register your e-voucher before you can use it.

*The membership registration period is up to three (3) months from the date of purchase. 

Via ONLINE

1. Visit the Online Card Registration Page
2. Fill in the Reference Number, which you’ll find in your purchase receipt email.
3. Provide the required personal details and click “Submit”
4. Once successfully registered, you will receive your Maxicare PRIMA Silver electronic health card (eCard) via email along with your policy and card number via SMS
5. You may avail of your unlimited consultations and laboratory tests as soon as you receive your Maxicare PRIMA Silver eCard.
6. Your Maxicare PRIMA Silver membership card will be delivered to your nominated address about 15 working days after your registration.

Reminders

• Present your Maxicare PRIMA Silver card or eCard along with 1 valid ID with a photo upon availing of the services offered in this healthcare plan
• No card or eCard, no availment
• Membership is valid up to 1 year from the date of activation
• The card is non-transferable once registered
